John Miall retired from the City of Asheville in 2005 after a 30 year career that spanned Human Resources, Benefits Management, and as Director of Risk Management. In 2000 he was name an “Honorary Member of the American Pharmacists Association (APhA.) In 2003 he received a “Benny Award” from Employee Benefit News for Outstanding Benefit Design.” In 2004 he was named “Public Risk Manager Of The Year by The Public Risk and Insurance Management Association (PRIMA.)
As Director of Risk Management for The City of Asheville, Miall held major operational and budget responsibility for city-wide self-funded liability programs including $16million program for general, auto, public officials, law enforcement, and dams liability; $10 million annual group benefits program; $2million worker's compensation program; and four separate retirement programs, two of them self-funded and self-administered. In 1997 John was one of a group of “visionaries” behind what became know as “The Asheville Project®.”

Ray Bullman joined the staff of the National Council on Patient Information and Education (NCPIE), in 1985. NCPIE, organized in 1982, is a non-profit coalition of diverse member organizations committed to improving communication of information on appropriate use of medicines to consumers and health care professionals. He has served as Executive Vice President since January 1995.
Under Mr. Bullman’s leadership, NCPIE, in August 2007 released the second in a series of comprehensive reports entitled, “Enhancing Prescription Medicine Adherence: A National Action Plan.” The report includes a series of 10 action steps for diverse stakeholder organizations to address to improve medication adherence. An initial NCPIE report on adherence improvement was released in 1996. In that same year, Mr. Bullman collaborated with the American Medical Association on the development of AMA’s “Guidelines for Physicians for Counseling Patients about Prescription Medications in the Ambulatory Setting.” Mr. Bullman is currently serving as the co-chair of the chronic conditions working group in a multi-stakeholder collaboration with the National Consumers League to develop / launch a national medication adherence campaign in 2011. |

John W. Walsh is the Co-founder and President of the COPD Foundation, a not-for-profit organization dedicated to developing and supporting programs which improve the quality of life through research, education, early diagnosis and enhanced therapy for persons whose lives are impacted by Chronic Obstructive Pulmonary Disease. He is also the Co-founder of the Alpha-1 Foundation (a research organization) and AlphaNet, Inc. (a unique not-for-profit disease management services company run by and for patients). He is a member of the NIH Director’s Council of Public Representatives and the NIH’s Council of Councils, is a member of the US COPD Coalition Executive Committee, Past-Chair of the International COPD Coalition (2006-2008), Past Chair (2005-2006) of the National Health Council’s Board of Directors, Past Chair and a member of the American Thoracic Society Public Advisory Roundtable (PAR), was the Presidential Appointee (2004-2005) of the American Thoracic Society’s Board of Directors and a Trustee of the Foundation of the American Thoracic Society (2006-2008). He regularly testifies before Congress and advisory groups as a patient advocate. Mr. Walsh was diagnosed with Alpha-1 genetic-related COPD in 1989. |

Dr. Mike Sokol serves as the Global Corporate Medical Director for Merck. He leads the Integrated Health Management department that is responsible for operating the company's employee health clinics, fitness centers, disability management programs, and occupational health activities. Mike also provides direction to Merck's health care benefits strategy and wellness initiatives, and he chairs the company's management team that addresses public health challenges to the employee population, such as pandemic flu.
In his prior roles at GlaxoSmithKline and Medco Health Solutions, Dr. Sokol provided subject matter expertise and consultative support to external customers. His areas of focus included population health and disease management, value-based benefit design, and health care quality improvement. He also conducted health services research by examining factors that influence medication adherence and its impact on clinical and financial outcomes. Dr. Cynthia Russell, program of research focuses on medication adherence in adult and older transplant recipients and dialysis patients. She has received funding for this program of research from a wide variety of sources and recently completed a John A Hartford-Atlantic post-doctorate fellowship (2005-2007).
Dr. Russell’s research program has been funded consistently since 2002 in amounts ranging from $10,000 to $150,000. Her funded research, published in refereed journals, has focused on medication taking beliefs among adult renal transplant recipients, patterns and predictors of medication nonadherence in adult and older renal transplant recipients, and intra-subject congruence of medication event monitoring system-generated medication adherence data.
She is currently piloting the continuous self-improvement intervention with renal transplant recipients. Her dissertation research tested a nursing intervention for those awaiting renal transplantation using a randomized controlled trial design. |

David Rose is a product designer, technology visionary, and serial entrepreneur. Currently David is Chief Executive Officer at Vitality, a company reinventing medication packaging with wireless technology.
Rose founded and was CEO of Ambient Devices where he pioneered glanceable technology: embedding Internet information in everyday objects like light bulbs, mirrors, refrigerator doors, digital post-it notes, and umbrellas to make the physical environment an interface to digital information.
In 1997 Rose patented online photo-sharing and founded Opholio. Before the Internet he founded Interactive Factory which creates interactive museum exhibits, educational software and smart toys, including the award-winning LEGO Mindstorms Robotic Invention System.

Derek Cothran, M.H.A., Senior Director, provides strategic direction to multiple clients for their specialty medications across a range of disease states. This includes the design and implementation of patient support programs including programs for the uninsured and underinsured with incorporated adherence components. He works closely with other AmerisourceBergen companies to integrate these programs across other identified needs such as specialty distribution and strategic consulting.
In addition, Mr. Cothran actively engages with manufacturers to respond to regulatory requirements such as Risk Evaluation and Mitigation Strategies (REMS) that impact product commercialization. In this role he has partnered with manufacturers to determine the appropriate services that are required for patients and providers to access therapy while adhering to the agreed-upon risk management plan.
He has also been a speaker regarding patient access issues, industry support service trends, reimbursement landscape changes and REMS.
Before joining Lash Group, Mr. Cothran worked at a healthcare accounting and consulting firm where he provided strategy and financial services to hospitals, health systems, physicians, and long-term care facilities. He specialized in service line and business plan development as well as long-term competitive strategies. Mr. Cothran received an M.H.A. and a B.S. in physiology from the University of South Carolina. |

Laura Cranston serves as the Executive Director of PQA, Inc. which is a pharmacy quality alliance established in April 2006, to improve health care quality and patient safety. PQA, as an alliance, works through a collaborative process with a broad range of stakeholders to reach a consensus strategy for measuring performance at the pharmacy and pharmacist-levels; collecting data in the least burdensome way; and reporting meaningful information to consumers, pharmacists, employers, payers, and other healthcare decision-makers to help make informed choices, improve outcomes and stimulate the development of new payment models.
Prior to her current position, Laura served as the Executive Director for the Institute for the Advancement of Community Pharmacy, an organization whose mission was to simply advance the practice of both independent and chain community pharmacy in the United States.
For over a decade, Laura served as the Vice President of Pharmacy Affairs for the National Association of Chain Drug Stores (NACDS), a trade organization with a membership base that operates over 30,000 retail community pharmacies. While at NACDS, she was responsible for developing policy and positions, in concert with NACDS chain corporations, on issues that included, but were not limited to: third party issues, state board of pharmacy regulations, DUR/counseling, training pharmacy technicians, and legislation to establish a Medicare prescription drug benefit. |

Jim Manfred received his pharmacy degree from Purdue University School of Pharmacy in 1977. Jim started with a small independent chain pharmacy where he managed a store for 3 years. He then went to work in hospital pharmacy before working for Walgreens for 16 years. Jim started his career with CHD Meridian at the Steel Family Pharmacy in Chesterton, Indiana in 2001. He was promoted to Assistant Vice President of Pharmacy Operations in 2003 and to Vice President of Pharmacy Operations in 2007. His responsibilities include the clinical oversight of forty-three employer sponsored pharmacies located in 18 different states, assisting sales in meeting with prospective clients and working with clients to maximize their pharmacy benefit program.
Jim is an executive member of Take Care Health’s Pharmacy and Therapeutics committee and is also a member of both the Primary Care Medical Advisory Board and the Occupational Health Advisory Board for Take Care Health.
Since 2006, Jim has supervised the implementation of all CHD Meridian’s pharmacies. Since the acquisition by Walgreens’, Jim has been tasked with leading the integration of the Take Care Pharmacy Program into Walgreens’ so that both types of pharmacy practice benefit from the experience and strengths of both companies. |

Peter Vasquez, MD graduated from Ohio State University College of Medicine, and completed Residency Training in and Board Certification in the specialty of Family Practice, in which he practiced for 15 years. He went on to complete Post-Graduate training in the field of Occupational and Environmental Medicine, gaining eligibility for Board Certification. He has served as the Medical Director for Disability Management for the North American division of BASF, the Area Medical Director for Hercules and Alliant Tech Systems, and an occupational medical consultant for employers such as USAA, UPS, Becton-Dickenson, Procter and Gamble, the Ross Division of Abbott Pharmaceuticals, and Owens Corning, and the Corporate Medical Director of MBI Occupational Health Care in Arizona.
Dr. Vasquez currently works for Take Care Health Systems, where he is the Western Regional Medical Director. Responsibilities include operational oversight, multistakeholder collaboration, and strategic planning. He serves on the boards of the directors of the Arizona Work Disability Prevention Association and the Western Occupational and Environmental Medical Association. |
